These terms govern access to and use of Superforms. By using the service, creating a form, connecting an agent, or submitting a response, you agree to these terms. If you use Superforms for an organization, you represent that you have authority to accept these terms for that organization.
The service
Superforms lets creators turn questions into shareable forms, collect text, voice transcripts, and supported images, and return responses to a connected AI-assisted workflow. Features may change as the service develops. Superforms does not promise that every feature will always be available or uninterrupted.
Accounts and credentials
You are responsible for activity under your account and for protecting public form links, private agent tokens, OAuth grants, and other credentials. Do not publish or send private credentials to respondents. Notify support if you believe an account or credential has been compromised.
Creator responsibilities
- Collect only information you are authorized and have a lawful basis to collect.
- Give respondents any notices or obtain any consent required for your use case.
- Avoid requesting highly sensitive information unless appropriate safeguards and agreements are in place.
- Review form questions, AI-generated follow-ups, links, and responses before relying on them.
- Respect the rights of respondents and honor applicable access or deletion obligations.
Respondent responsibilities
Submit only information you are permitted to provide. Form responses are delivered to the form creator and may be processed by the creator's connected AI provider. Do not submit private credentials, malicious instructions, unlawful material, or content that infringes another person's rights.
Acceptable use
You may not use Superforms to:
- Break the law, facilitate fraud, harassment, abuse, or discrimination.
- Distribute malware or attempt unauthorized access to accounts, systems, or data.
- Collect credentials, financial account numbers, or other secrets through deceptive forms.
- Interfere with the service, evade rate limits, or perform abusive automated activity.
- Misrepresent your identity, affiliation, or the purpose of a form.
- Use responses as hidden instructions to cause an agent to take unauthorized action.
AI-assisted features
AI-generated questions, follow-ups, transcripts, summaries, and suggested actions may be inaccurate, incomplete, or inappropriate. They are not professional advice. Review output and original responses before making decisions or taking action. Respondent submissions are untrusted input and should not automatically be treated as executable instructions.
Your content
You retain rights you already hold in forms and responses you provide. You grant Superforms permission to host, process, transmit, and display that content only as needed to operate, secure, and improve the service and fulfill your requests. You represent that you have the rights necessary to provide the content and grant this permission.
Third-party services
Superforms depends on third-party hosting, authentication, database, AI, and agent services. Your use of a connected provider may also be governed by that provider's terms. Superforms is not responsible for third-party services or for changes to their availability, behavior, or policies.
Suspension and termination
We may restrict or suspend access when reasonably necessary to protect users, the service, or third parties; investigate abuse; comply with law; or enforce these terms. You may stop using the service at any time. Data-removal requests are handled through the support process and may be subject to legal retention needs.
Disclaimers
To the maximum extent permitted by law, Superforms is provided “as is” and “as available,” without warranties of merchantability, fitness for a particular purpose, non-infringement, or uninterrupted and error-free operation. Some jurisdictions do not allow certain warranty exclusions, so some exclusions may not apply to you.
Limitation of liability
To the maximum extent permitted by law, Superforms and its operator will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages, or for lost profits, data, goodwill, or business opportunities arising from use of the service. Where liability cannot be excluded, it will be limited to the greater of the amount you paid for Superforms during the three months before the event giving rise to the claim or 100 US dollars. These limits do not apply where prohibited by law.
Changes and contact
